I am fully aware and hereby confirm that my daughter is applying for the OMG Institute Modeling Scholarship and wishes to explore the opportunity of pursuing a career as a runway model. I have discussed this opportunity with my daughter, reviewed the materials provided, and give my informed, voluntary consent for her to submit the application and participate in the selection process.

This consent is limited to the application and selection process.It does not constitute consent to any subsequent contract, photo shoot, travel, commercial engagement, or use of my daughter's image and likeness. Any such activity will require a separate written agreement signed by me in advance.

My daughter has direct, unrestricted access to a mobile phone to reach me at any time. I expect to be informed of any in-person meeting, audition, or evaluation involving my daughter at least 48 hours in advance and to be invited to attend. Please do not hesitate to verify this consent with me by phone or email before proceeding.
